their self-care management plans for
a specific chronic illness with which
they have been diagnosed. At present
HyHealth covers 21 different chronic
disease conditions. A participant
may actually have multiple diagnoses
which are referred to as co-morbidities.
Hence, when viewing educational information
about a specific disease, participants
with co-morbidities must understand
that their situation may be more complicated
than represented by a specific disease
discussion. This is why it is important
that the self-care plan developed
by the participant and the HyHealth
nurse be followed as closely as possible.
The HyHealth nurse reviews all treatment
and pharmacy data as well as the diagnosis(es)
participants have in working with
them to formulate a good self-care
plan.
