HyHealth's Personal Self-Care Plan Helps Control Rising Healthcare Cost
Participants monitor their own pertinent health data and report to their HyHealth nurse. Each participant is assigned a primary contact to work with them while participating in HyHealth. They begin by developing a personal self-care plan individualized to deficits regarding their condition. One-on-one feedback is provided regarding their personal self-care plan and modifications are made as needed. In this way, the physician's plan of treatment is enhanced by HyHealth. Participants who are involved in their health care and adhere to treatment plans can improve health, delay complications from chronic conditions, and do their part in controlling rising health care costs.
