When you establish a relationship with MyHealth you are asked to share personal information used for assessment, and to enable us to serve you and administer our product. This information comes to us from applications and other forms you submit, and includes:
- Information we receive from you on Health Risk Evaluations and biometric screenings;
- Information we receive through interactions with you related to Wellness events, Lifestyle and/or Disease Management interventions;
- Data we receive from Third-Party Administrators, Pharmacy Benefit Managers, and others who collect and aggregate health data; and
- Your contact information, such as address telephone numbers and e-mail addresses.
MyHealth will use this information only to enable us serve your needs and objectives. The information you share with us is important to you, and you can expect that we will protect its privacy and use, Our professionals and staff have access to your information only when it is necessary for them in the administration of our product and services for you. All of our employees are strictly held to this privacy policy. Also, your private information is stored on secure sewers that are not accessible by any third-party.
Our Disclosure of Your Information to Others
MyHealth does not disclose any of your private information to any third-party except as it may be required by law, regulation or a court order. For example, in order to comply with legal or government requirements, we may be required to provide government agencies or law enforcement authorities with information. Also, federal and state laws allow persons involved in lawsuits or other legal proceedings the ability to obtain records and information using valid legal process and procedures. ln these circumstances we share only the required information. We may also disclose your personal information when it is necessary to protect our own legal rights.
Except as necessary in the performance of our services, it is not the policy of MyHealth to share your information with any third-party that may seek to offer you products or services. However, the
MyHealth web site contains links to other web sites, and those third-parties may seek to offer you products or services. It is our policy not to provide information to these third-parties, but MyHealth accepts no responsibility for the protection of private information you may supply to them. Keeping Information Current
MyHealth is committed to keeping your personal information secure, accurate and current. You should provide us with any updates and changes to your personal information. Also, in the event your password for access to the MyHealth web site is lost, stolen or compromised in any way, you must notify us immediately so that we can cancel your user name and/or password, issue you a new user name and password, and update our records accordingly.
Opting Out of Information Sharing
Except as described above, MyHealth will not share any information with non-related third—parties. By entering into a relationship with MyHealth you consent to the collection and use of personal information as described in this Privacy Policy. This policy reflects MyHealth’s current business practices, and is subject to change and update. ln the event of a change, a revised policy will be sent to you or otherwise made available. To the extent any change affects our policy in regard to disclosure of information to third-parties; you may in that event elect to “‘opt-out" as described above. MyHealth will provide you with a copy of our Client Privacy Policy annually.
